Almost forty years ago David Ramsay devised a sy stems based model for the investigation of industr ial problems and accidents. The methodology\, Kelv in TOP-SET®\, is now taught to industrial clients in fifty five countries.

Serious investigations have been l ed or facilitated by David Ramsay and his team in over thirty countries at the latest count. These i nvestigations covered many sectors from offshore o il and gas\, marine\, rail civil engineering\, to chemicals and explosives. Many of these investigat ions included injury and loss of life and major ca pital loss including the 2010 Deepwater Horizon Di saster conducted on behalf of the rig owners.
