Application, Direct Debit and Gift Aid Forms

If you have already completed our "Connect with Us" online form, then please click here to download our application form. The form is a short MS-Word document, to which we ask you to attach your curriculum vitae at the end. When completed, you should send the combined document to your sponsor who will then liaise with your seconders, the Clerk and Membership Chair.

If you have yet to complete our initial "Connect with Us" online form, please click here to open it in a new tab. This form sets up some initial information within our membership systems and ensures that we are complying with GDPR legislation.

We manage all membership payments by Direct Debit. Please therefore click here to download a Direct Debit Mandate which should be printed and then completed and signed by hand, and returned to The Clerk at our office address.

Similarly, as contributions to our Charitable Fund are eligible for Gift Aid, please download this declaration, then complete (electronically) and return it to The Clerk.
